ConservCare utilizes a "primary care coordination model" of patient management. When one of our Care Coordinators is assigned a case, they will be the primary point of contact for that patient/claimant for their entire length of therapy. Although each Care Coordinator works with an entire team of support professionals, they will be your single point of contact, even for additional services, status report requests, and billing review requests.
